Whenever I saw a doctor last summer, I had to fill out that silly form. I saw
the OBgyn, internest, eye doctor, allergist and Dexa doctor. I had blood
tests, mammogram, and the nasty test. I had to sign those forms wherever I
went. When I asked for the report at the Dexa scan doctor's office, she said
I had to ask my doctor. I looked at that stupid form and she said, ok, but I
would have to pay $2.00. Where I live everyone is required to give you
information you want.
